conclusions since most of the symptoms of metabolic diseases in newborns are same as the signs of sepsis or other common diseases of this period, and also early diagnosis and treatment of these illnesses, especially in diseases such as hyperglycemia, can be influential in neural function of babies, it calls for serious attention and clinical vision of the doctors to stop mortality caused by such diseases by early consideration of serious complications. introduction glycine encephalopathy, also known as non-ketotic hyperglycinemia (nkh) represents a disorder characterized by elevated concentrations of glycine in all body tissues, especially in plasma and cerebrospinal fluids (csf). nkh is caused by deficiency in the glycine cleavage system (gcs). most glycine encephalopathy cases occur during the neonatal period. the current report presents a case of nkh neonatal intractable seizures. evaluating a sick neonate who presents with hypotonia, encephalopathy, and seizures is a diagnostic challenge; a high index of suspicion for timely diagnosis and treatment could prevent severe complications. case presentation the patient was a four-day-old baby girl in iran admitted to the hospital due to hypotonia, lethargy and seizures. on the third day after discharge, the baby gradually lost the ability to suck milk and got sleepy and finally suffered from organ jump and seizures. sepsis and antibiotics phenytoin tests were conducted and in metabolic testing, high level of glycine in blood and urine was reported and the patient was treated with sodium benzoate dextromethorphan and l-carnitine, after the start of the treatment, seizures and neurologic symptoms and alertness of the baby improved. the patient was discharged by continuing treatment after two weeks with seizure control, improved sucking reflexes and grasp.

Non-ketotic hyperglycemia unmasks hemichorea

BACKGROUND Chorea can be caused by a variety of diseases, including neurodegenerative disorders, vascular events, toxic-metabolic states, and immunologic and infectious diseases. We describe a patient who presented with hemichorea as the initial manifestation of Diabetes Mellitus (DM) and responded partially to the glycemic control. CASE REPORT A 63-year-old, healthy Hispanic man with no prio...

Hemichorea-Hemiballism Secondary to Non-Ketotic Hyperglycemia

Non-ketotic hyperglycemia is an unusual and rare cause of hemichorea-hemiballismus. Correction of the hyperglycemia usually results in total resolution of the signs and symptoms. We present the case and medical imaging findings of a 66-year-old female who presented with steadily worsening choreiform and ballistic movements of the right upper and lower extremities over a 2-week period. Her serum...

Complex partial seizures and aphasia as initial manifestations of non-ketotic hyperglycemia. Case report.

We describe a case of non-ketotic hyperglycemia (NKH), heralded by complex partial seizures and aphasia of epileptic origin, besides versive and partial motor seizures. This clinical picture was accompanied by left fronto-temporal spikes in the EEG. The seizures were controlled by carbamazepine only after the control of the diabetes. A month later, carbamazepine was discontinued. The patient re...

Reflex seizures and non-ketotic hyperglycemia: an unresolved issue

Reflex seizures are a rare form of epilepsy, the pathogenesis of which is unclear. They have been reported in the setting of non-ketotic hyperglycemia (NKH) and are considered to be neuroendocrine in origin. We report a diabetic patient with movement-induced seizures whose presentation suggests that brain ischemia may be the precipitating event in focal seizures seen in the setting of NKH. We r...

A Case Series of Hemichorea due to Non Ketotic- Hyperglycemia with Unique MRI Brain Finding

Hemi chorea generally occurs due to a stroke, neoplasm or granuloma in the contra lateral basal ganglia [1]. Metabolic derangements especially hyperglycemia can also cause focal neurological dysfunction like hemiparesis, focal seizure [2,3]. Recently there have been case reports of hemichorea reported due to non-ketotic hyperglycemia with characteristic MRI findings predominantly in the Asian p...

Diabetic non-ketotic hyperglycaemia presenting as chorea--a case report.

We report a patient with hyperosmolar non-ketotic hyperglycaemia who presented with chorea and septic arthritis on his knee. The chorea resolved completely and quickly with correction of the metabolic disturbance, only to return just as quickly when his metabolic disturbance subsequently deteriorated as a result of overwhelming septicaemia, suggesting coexisting cerebral ischaemia, although the...
